Feedback Popup on Cancelation

Hello,

we would like to have a feature, where if a User would cancel the subscription a Popup would be triggered.
This popup should gather feedback on why he canceled the subscription.

This would help to gather information and improve the website, so that less user cancel.

Would be awesome, if the fields for the Popup could be customizable.
